Informational Letter for Bigs & Parents

Agreement to Abide by the Overnight Visits Policy
(to be completed ONCE by each match.
Matches made since June 2010 will have completed this Agreement as part of
the match process. Matches made before June 2010 will need to complete and
return this agreement to their Match Support Specialist.)

Overnight Consent Form
(to be completed and signed by parent/guardian before EVERY overnight)

(to be completed before EVERY overnight)

EVERY overnight must be approved IN ADVANCE
by your Match Support Specialist . Please recognize that
your Match Support Specialist may work a part-time or flex schedule,
and you may need to allow several days’ notice in order to get pre-approval.
